Wedding Officiant Request Form

Thank you for inquiring about our Pastor's availability to officiate your upcoming wedding ceremony. Please fill out this form and we will be in touch soon.

Please note the following rules and limitations:

Blackout Dates: We are generally unavailable to officiate weddings on holidays unless a special exception is made.
Travel: For weddings that require travel of 25 or more miles from Lafayette, additional planning and compensation might be required.
Fees: An officiants fee of $250 is due by cash or check on the day of the wedding.
Rehearsal Dinner and Ceremony Details: Please note that all details regarding the rehearsal dinner time/location and ceremony time/location must be finalized prior to requesting officiates services. This is to honor and respect our officiates and their time.
Deadline: Officiant request must be made at least 3 months prior to the wedding. This is non-negotiable as to give time for pre-marital counseling and scheduling.
